Position Overview:

The Clinical Transition Specialist is tasked with delivering clinical education and training to facilitate the successful transition of patients into the Option Care Health service model.

This role involves collaboration with the sales team to enhance the number of patients transitioned to our care model.

Key Responsibilities:

  • Assess, educate, and train patients, caregivers, and facility personnel on how Option Care Health services and products will be implemented in alternative settings, at home, or virtually to ensure a seamless transition.
  • Conduct thorough patient assessments to evaluate their suitability for the Option Care Health service model.
  • Collaborate with the sales team to identify educational needs related to clinical transitions and develop strategies to boost patient transition rates.
  • Uphold the confidentiality of patient and proprietary information while adhering to legal standards for safeguarding sensitive data.
  • Document educational interactions accurately using Option Care Health applications and technology.

Supervisory Responsibilities:

This position does not entail supervisory responsibilities.

Basic Education and Experience Requirements:

  • Must possess an active and unrestricted Registered Nurse (RN) license.
  • A minimum of two years of experience in the healthcare sector is required.

Essential Qualifications:

  • Proven ability to establish and maintain relationships with individuals at all organizational levels and with external vendors.
  • Familiarity with standard practices for all services offered and current relevant standards (e.g., ACHC, URAC standards).
  • Experience in delivering customer service to both internal and external clients, ensuring quality service standards and evaluating customer satisfaction.
  • Proficient in basic computer skills, including internet usage, email, Microsoft Word, Excel, and PowerPoint.
  • Ability to identify operational challenges and propose effective strategies for process improvement.
  • Access to reliable transportation for travel to care facilities, home visits, and various hospitals.
  • Willingness to obtain nursing licensure in additional states as needed.
  • Capable of planning, organizing, and delivering presentations.

Travel Requirements:

This role requires 100% local travel to partner facilities, community hospitals, and medical practice offices to promote Option Care Services, process referrals, and provide live education and training support to patients, caregivers, and referral sources.

Preferred Qualifications:

  • Experience in home or alternate site infusion or discharge planning.
  • Proven track record in developing service provider partnerships.

Salary will be determined based on the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and market alignment.
