Position Overview:
The Clinical Transition Educator plays a pivotal role in delivering clinical education and training to facilitate the seamless transition of patients into the Option Care Health service model.
Key Responsibilities:
  • Assess, instruct, and train patients, caregivers, and facility personnel on the implementation of OCH services and products, whether in an alternative site, at home, or through virtual platforms, ensuring a successful transition.
  • Conduct thorough patient evaluations to ascertain their suitability for the OCH care delivery model.
  • Collaborate with the OCH sales team to identify educational needs regarding clinical transitions and develop strategies to enhance patient transition rates.
  • Safeguard the confidentiality of patient and proprietary information while adhering to legal standards.
  • Document educational interactions accurately using OCH applications and technology.
Supervisory Duties:

This role does not include supervisory responsibilities.

Essential Qualifications:
  • Must possess an active and unrestricted Registered Nurse (RN) license.
  • A minimum of two years of experience in the healthcare sector is required.
Preferred Skills:
  • Proven ability to build and maintain relationships across various organizational levels and with external partners.
  • Familiarity with standard practices for all services offered and current relevant standards (e.g., ACHC, URAC).
  • Experience in delivering exceptional customer service and evaluating customer satisfaction.
  • Basic proficiency in PC applications, including internet, email, Microsoft Word, Excel, and PowerPoint.
  • Ability to identify operational challenges and propose effective solutions.
  • Access to reliable transportation for local travel to care facilities and hospitals.
  • Willingness to obtain nursing licensure in additional states as needed.
  • Strong organizational and presentation skills.
Travel Requirements:

This position requires 100% local travel to partner facilities, community hospitals, and medical offices to promote Option Care Services and provide live educational support.

Compensation:
The salary for this position will be determined based on the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and market alignment.
