Registered Nurse Care Coordinator

Starting Salary Range: $38 - $42.50 based on experience

  • Opportunities for both Day and Night shifts
  • Additional compensation of $1.25/hour for Weeknight shifts and $3.00/hour for Weekend shifts
  • This position is based on-site in a designated location. (Not available for remote work) There is no direct patient interaction.
  • A retention incentive of $4,000.00 is being offered.

POSITION OVERVIEW


The Nurse Care Coordinator will play a pivotal role in connecting various mobile resources, ensuring they are integrated and coordinated to effectively address patient requirements.

Responsibilities include telephonic patient assessment utilizing established evidence-based guidelines to facilitate care navigation throughout the healthcare system. The role encompasses telemonitoring and patient education initiatives, with a focus on coordinating team care in a virtual environment. Additional responsibilities may include leadership and mentoring tasks as assigned.

KEY RESPONSIBILITIES

  • Exhibit proficiency as a Registered Nurse
  • Conduct telephonic assessments and guide care based on evidence-based protocols.
  • Provide patient education across the healthcare spectrum.
  • Engage in telemonitoring activities to promote healthy patient behaviors.
  • Demonstrate strong communication skills and collaborate effectively with team members, patients, caregivers, referral sources, physicians, and facility staff.
  • Show capability in learning and utilizing required software and computer applications.
  • Participate in initiatives that enhance the Integrated Healthcare Program and the overall organization.
  • Act as a facilitator and educator for the Integrated Healthcare training program.
  • Manage time, resources, and documentation efficiently.
  • Deliver exceptional customer service.
  • Undertake additional duties as required.

QUALIFICATIONS

Experience:

  • Possession of a Compact RN License
  • Minimum of 5 years of RN experience in Emergency or Acute Care environments
  • Familiarity with clinical data collection and ethical standards.

Education:

  • Graduate from an accredited nursing program
  • Master’s or Bachelor’s degree in Healthcare Administration, Public Health, or a related field focused on community or disease education;

Skills:

  • Strong verbal, written, and interpersonal communication abilities.
  • Nursing skills aligned with generally accepted practice standards. Excellent interpersonal skills.
  • Ability to comply with established educational quality standards.
  • Proficient in basic computer applications and functionalities.
  • Bilingual proficiency is a plus.
  • Demonstrated skills in teaching and educational facilitation.

CREDENTIALS/LICENSES:

  • Maintain an active RN Compact state Nurse License
  • Current CPR certification.
