Responsibilities- Provide skilled pediatric home care to patients with complex medical needs
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with healthcare teams to ensure seamless care coordination
- Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies
- Administer medications, perform medical procedures, and provide wound care as needed
- RN/LVN with a current valid license to practice as issued by the State Board of Nursing
- Current CPR certification
- Experience working with pediatric patients with complex medical needs
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
