Position Overview

The Clinical Counselor plays a pivotal role in delivering comprehensive therapeutic services at Lennox CMHC, Inc., ensuring the highest quality of care and support for clients throughout their recovery journey.

Key Responsibilities:

  • Conduct thorough screenings and evaluations of potential clients to assess their suitability for the facility.
  • Provide essential referral options as necessary, facilitating a smooth transition into the program.
  • Engage with newly admitted clients, offering support and guidance in understanding the program's philosophy.
  • Perform detailed Psychosocial Assessments to identify client needs and summarize findings in a cohesive written format.
  • Maintain on-call availability during weekends and nights for client support.
  • Lead both group and individual therapy sessions in accordance with established program standards.
  • Document client progress and participation meticulously on a daily basis.
  • Develop and implement personalized recovery plans with measurable goals tailored to each client.
  • Utilize information from various assessments to create individualized recovery plans within 24 hours of admission.
  • Regularly review and update recovery plans through written documentation during clinical staff meetings.
  • Assist clients with discharge planning, including providing necessary referrals and directing them to continuing care programs.
  • Prepare and deliver educational lectures on addiction and relapse prevention as required.
  • Utilize appropriate teaching aids to enhance the learning experience during presentations.
  • Maintain clear communication regarding client progress with facility staff and referral sources.
  • Ensure accurate and timely documentation in client charts, including progress notes and recovery plan updates.
  • Transport clients to off-site appointments as needed, ensuring their safety and well-being.
  • Foster professional relationships with referral sources and maintain open communication with facility staff.
  • Participate in all required meetings, trainings, and performance improvement initiatives.
  • Act as a representative of the facility within the community and educational institutions as directed.
  • Monitor and evaluate work performance to identify areas for improvement and quality assurance.
  • Adhere to all facility policies and procedures, demonstrating a commitment to ethical standards and client dignity.

Administrative Duties:

  • Engage in quality improvement initiatives as assigned.
  • Coordinate with insurance and managed care companies for client services.
  • Facilitate community referrals and ancillary services for clients, including transportation coordination.
  • Ensure safe transportation of clients to community meetings and appointments as required.
  • Participate in ongoing training and supervision meetings to enhance professional development.
