POSITION SUMMARY:

The Transition Services Coordinator I is responsible for overseeing the activities of the Transition Services Training program for BAC participants. This includes facilitating goal-setting, managing work assignments, tracking progress, and ensuring effective communication with Individuals with Disabilities (IWD) to foster their growth and learning in a professional setting. The role involves organizing daily tasks and lessons to maximize efficiency and productivity. The work is conducted independently with limited supervision, allowing for moderate discretion and independent judgment. This position typically requires the interpretation and handling of more complex, less clearly defined issues.

KEY RESPONSIBILITIES:

The following functions are essential to this role. Additional duties may be assigned based on organizational needs:

  • Deliver various Transition services tailored to participants based on their individual skill levels and in alignment with the Transition team’s objectives.
  • Assist in the planning and execution of program activities; evaluate their effectiveness and recommend enhancements as necessary.
  • Maintain accurate documentation of participant goals and progress; submit reports to program administrators and other authorized personnel as scheduled.
  • Act as a liaison between participants and their families/community members to ensure timely and accurate communication.
  • Collaborate with transition services instructors to develop training methodologies and monitor participant achievements.
  • Complete all implementation plans and reports accurately and on time; ensure submission to relevant program administrators and authorized recipients as required.
  • Perform physical tasks associated with the role, including bending, lifting, and walking for extended periods.
  • Assist with personal care tasks, including hygiene, feeding, and mobility support.

MINIMUM REQUIREMENTS:

  • High School Diploma or GED.
  • A minimum of four years of experience in education or working with individuals with disabilities (IWD).
  • An equivalent combination of education, certification, training, and/or experience may fulfill the above requirements.
  • Must meet Florida Department of Children and Families (DCF) Level 2 background screening criteria.
  • Certification in first aid, CPR, and AED, along with eight hours of continuing education units (CEUs) as specified by the Agency for Persons with Disabilities (APD).
  • Additional formal industry certifications may be required based on specific assignments.
  • Must successfully pass a pre-employment drug screening.

PREFERRED QUALIFICATIONS:

  • Bachelor's Degree in a related field.
  • Experience working with IWD.
  • Background in Behavioral Management.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Understanding of principles, practices, techniques, and professional standards in habilitation services for individuals with disabilities (IWD).
  • Familiarity with community organizations and social service programs.
  • Knowledge of employee support service principles and procedures.
  • Awareness of the Health Insurance Portability and Accountability Act (HIPAA).
  • Proficiency in using Microsoft Office suite and relevant department-specific software.
  • Ability to manage confidential employment information with discretion.
  • Capability to read, analyze, and interpret program-related laws and regulations, establishing necessary work processes.
  • Ability to prioritize multiple tasks to meet deadlines.
  • Critical thinking skills to propose solutions and process improvements.
  • Ability to build and maintain effective working relationships with colleagues and stakeholders.
  • Consistent attendance and punctuality for scheduled work hours.
  • Effective communication skills, both verbal and written.

TRAVEL:

No travel is anticipated for this position.

WORK AUTHORIZATION/SECURITY CLEARANCE:

PHYSICAL DEMANDS:

Depending on the assigned functional area, tasks may involve routine moderately physically demanding work, including lifting and moving moderately heavy objects (up to 40 pounds). Tasks requiring significant weight movement may necessitate assistance or the use of proper techniques and equipment. Some tasks may involve climbing, stooping, kneeling, or crawling, which can be performed with or without reasonable accommodation.

ENVIRONMENTAL CONDITIONS:

Tasks are primarily performed indoors, with minimal exposure to adverse environmental conditions (e.g., dirt, heat, cold, rain, fumes). Occasional incidental outdoor exposure may occur.

SENSORY REQUIREMENTS:

Some tasks require manual dexterity, as well as visual and auditory acuity.

AVAILABILITY:

Full-time; Monday to Friday

SALARY: $17 per hour
