POSITION SUMMARY:

The Transition Services Coordinator I is responsible for overseeing the Transition Services Training program activities for BAC participants. This includes facilitating goal-setting, managing work assignments, generating progress reports, and maintaining effective communication with Individuals with Disabilities (IWD) to foster their growth and learning within a professional environment. The role involves scheduling daily activities and lessons to maximize efficiency and productivity. The position is performed independently with limited supervision, allowing for moderate discretion and independent judgment in addressing complex and less clearly defined issues.

KEY RESPONSIBILITIES:

The essential functions listed below represent the tasks performed in this role. Additional duties may be assigned based on organizational needs.

  • Delivers various Transition services tailored to participants' individual skill levels and aligned with the Transition team plan.
  • Assists in planning and implementing programs; evaluates effectiveness and recommends improvements as necessary.
  • Maintains comprehensive documentation on participant objectives and progress towards their goals; submits this documentation to program administrators and other authorized parties as scheduled.
  • Acts as a liaison between participants and their families/community members to ensure timely and accurate communication.
  • Collaborates with transition services instructors to develop training methodologies and monitor participant progress.
  • Completes all implementation plans and reports accurately and punctually; submits them to program administrators and other authorized recipients as scheduled.
  • Handles the physical demands of the job, including bending, lifting, pushing, pulling, reaching, and walking for extended periods.
  • Performs personal care tasks, which may include assistance with hygiene, grooming, toileting, feeding, transitioning, and mobility.

SUPERVISORY RESPONSIBILITY:

None

MINIMUM REQUIREMENTS:

  • High School Diploma or GED.
  • A minimum of four years of experience in education or working with individuals with disabilities (IWD).
  • An equivalent combination of education, certification, training, and/or experience may fulfill the above requirements.
  • Must meet Florida Department of Children and Families (DCF) Level 2 background screening criteria.
  • Certification in first aid, cardiopulmonary resuscitation (CPR), and automated external defibrillator (AED) is required, along with eight hours of continuing competence units (CCUs) as specified by the Agency for Persons with Disabilities (APD).
  • Additional formal industry certifications may be required based on the area of assignment.
  • Must successfully pass a pre-employment drug screening.

PREFERRED QUALIFICATIONS:

  • Bachelor's Degree in a related field.
  • Experience working with IWD.
  • Knowledge in Behavioral Management.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Understanding of principles, practices, techniques, and professional standards in habilitation services for individuals with disabilities (IWD).
  • Familiarity with community organizations and social service programs.
  • Knowledge of employee support service principles and procedures.
  • Awareness of the Health Insurance Portability and Accountability Act (HIPAA).
  • Proficiency in using Microsoft Office suite and relevant department-specific software.
  • Ability to manage confidential employment information with discretion.
  • Capability to read, analyze, and interpret program-related laws and regulations, establishing necessary work processes and procedures.
  • Ability to prioritize multiple tasks to meet deadlines.
  • Critical thinking skills to identify solutions and propose process improvements.
  • Ability to establish and maintain effective collaborative relationships with colleagues and stakeholders.
  • Consistent attendance and punctuality for the designated work schedule.
  • Effective oral and written communication skills.

No travel is anticipated for this position.

PHYSICAL REQUIREMENTS:

Tasks may involve routine performance of moderately physically demanding work, including lifting, carrying, pushing, and/or pulling moderately heavy objects (up to 40 pounds). Moving significantly heavy objects may require assistance and/or proper techniques and equipment. Tasks may involve some climbing, stooping, kneeling, crouching, or crawling, with or without reasonable accommodation.

ENVIRONMENTAL REQUIREMENTS:

Tasks are typically performed indoors, with minimal exposure to adverse environmental conditions (e.g., dirt, heat, cold, rain, fumes). Occasional incidental outdoor exposure may occur.

SENSORY REQUIREMENTS:

Some tasks necessitate manual dexterity, along with visual and auditory acuity.

Full-time; Monday to Friday
