Position Summary

The Client Care Specialist collaborates with support staff on designated grants and programs within the organization. This role is pivotal in employing a collaborative and coordinated approach to connect participants with suitable health and social services tailored to their unique needs and treatment objectives. A thorough understanding of community resources and effective communication with professionals, providers, and participants is essential for successfully linking participants to services.

The Client Care Specialist's primary goal is to provide support services that enhance self-efficacy, self-advocacy, essential life skills, coping mechanisms, self-management of biopsychosocial needs, benefits, resources, and reintegration into the community. This is a full-time role dedicated entirely to care coordination and case management for grant participants.

Key Responsibilities
  • Maintain registration or certification with a state-approved AOD certifying body, required upon hire.
  • Work alongside a multidisciplinary treatment team to assess participants, addressing challenges in areas such as Housing, Employment, Education/Training, Financial, Legal, Sobriety/Social Support, SUD Treatment, Family/Social, and Medical/Psychiatric.
  • Conduct assessments and care coordination/linkages in various settings, including in-home and on-site.
  • Assist primary counselors with discharge planning from the moment of admission for all assigned participants, covering aspects like housing, employment, medical/psychiatric needs, financial matters, and aftercare.
  • Provide high-quality referrals and linkages for necessary resources and services as identified in the Treatment Plan, actively working to reduce barriers and ensure participants have genuine access to required services.
  • Serve as a liaison between health and human service providers to facilitate appropriate information exchange and ensure successful connections to resources/services.
  • Coordinate care with community agencies, assisting participants with eligibility, documentation, and applications necessary for accessing services/resources.
  • Support participants in transitioning within the SUD system of care, referring them to appropriate levels of care as needed.
  • Follow up with participants to confirm completion of scheduled appointments and linkages.
  • Collaborate and communicate effectively with team members regarding participant behavior and provide regular updates on progress.
  • Identify new and existing community resources that may benefit participants.
  • Engage in outreach to develop relationships with other providers and organizations that can support program participants in their recovery journey.
  • Communicate professionally with participants, staff, and other service providers through various channels, including progress notes and reports.
  • Provide individual counseling, group facilitation, and crisis intervention as part of a multidisciplinary team when necessary.
  • Maintain accurate and timely documentation of care coordination and other services provided.
  • Develop and submit reports regarding services delivered, clients served, outcomes of referrals, and retention rates as required.
  • Exhibit a comprehensive understanding and support of the philosophy and objectives of CLARE|MATRIX.
  • Conduct oneself professionally in terms of appearance, language, attitude, and behavior.
  • Adhere to policies regarding professional ethics.
  • Participate in training and relevant educational workshops.
  • Attend all required staff meetings.
  • Perform additional tasks and responsibilities as assigned by the supervisor or designee.
Qualifications
  • Registration or Certification with a state-approved AOD certifying body, required upon hire.
  • A genuine passion for working with individuals facing substance use disorders, employing a compassionate and person-centered approach.
  • Preferred: One year of experience in social services or a related field.
  • Strong oral and written communication skills.
  • Professional demeanor and appearance.
  • Ability to work independently and collaboratively within a team.
  • Capacity to follow directions effectively.
  • Ability to engage with diverse groups of clients, staff, and community members.
  • Organized, detail-oriented, and a creative problem-solver.
  • Proficient in computer skills and familiar with Microsoft products such as Outlook, Word, and Excel.
  • A valid driver's license is required.
Physical Demands

This role requires effective communication skills, including speaking and listening. The employee will often sit and use their hands and fingers, with occasional requirements to stand, walk, reach, climb, or balance. Close vision is necessary for this position.
