Job Title: Street Health Outreach Behavioral Health Case Manager

Tiburcio Vasquez Health Center is seeking a highly skilled and compassionate Street Health Outreach Behavioral Health Case Manager to join our team. As a key member of our Street Health Outreach program, you will play a vital role in providing essential services to individuals experiencing homelessness and struggling with chronic health and mental health needs.

Job Summary:

The Street Health Outreach Behavioral Health Case Manager will work directly with participants to conduct behavioral health assessments, provide brief interventions, and support clients in navigating mental health and substance use treatment systems from a harm reduction and trauma-informed approach. This is a grant-funded position, and employment is subject to the availability of grant funding.

Responsibilities:
  • Conduct behavioral health assessments and provide brief interventions to clients
  • Support clients in navigating mental health and substance use treatment systems
  • Maintain a caseload of clients who require more in-depth case management services
  • Provide outreach and engagement, health services, and triage to clients
  • Collaborate with Housing Resource Centers to provide comprehensive services to clients
  • Build relationships with clients over time through consistent visits and outreach
  • Screen and identify individuals with medical and behavioral needs
  • Prioritize clients with high medical and behavioral health needs for intensive visits
  • Participate in regular meetings with Alameda County Healthcare for the Homeless
  • Provide culturally competent, trauma-informed, and harm reduction-based services
  • Conduct regular outreach and engagement to build trust with clients
  • Respond to immediate survival needs as prioritized by clients
  • Provide assessments and administer screening tools as needed
  • Provide crisis assistance and brief behavioral health interventions
  • Link clients to community services as needed
Requirements:
  • High School Diploma (or G.E.D.) required
  • Basic Life Support (BLS)/CPR certification required
  • Valid current driver's license and individual automobile insurance required
  • Knowledge of principles and practices of medicine and allied services
  • Ability to build trust and respect with clients and colleagues
  • Ability to identify potential escalating situations and safety risks
  • Good problem-solving skills, including creativity and resourcefulness
Qualifications:
  • Bilingual in English/Spanish preferred
  • Minimum one year of field experience working with people with serious mental illness, individuals with a dual diagnosis, and/or the homeless population
  • Experience with street outreach desirable
  • Knowledge of Psychiatric Disorders and ability to implement harm reduction, psychosocial rehabilitation, strength-based case management, and motivational interviewing
