Job Summary

We are seeking a highly skilled and compassionate Outreach Specialist to join our team at Heluna Health. As a key member of our San Francisco Homeless Outreach Team (SFHOT), you will play a critical role in engaging and stabilizing individuals experiencing homelessness, connecting them with essential services, and advocating for their needs.

Key Responsibilities
  • Conduct outreach and engagement activities in collaboration with the Healthy Streets Operations Center (HSOC) Encampment Resolution Team (ERT) to support individuals experiencing homelessness.
  • Collaborate with various City of San Francisco departments, including the San Francisco Fire Department, San Francisco Police Department, Department of Public Works, and Department of Emergency Management, to ensure a comprehensive approach to addressing homelessness.
  • Provide support and advocacy for individuals experiencing homelessness, connecting them with shelter referrals, medical services, and other essential resources.
  • Develop and maintain relationships with community partners, service providers, and neighborhood stakeholders to ensure a coordinated response to homelessness.
  • Participate in training and professional development opportunities to enhance skills and knowledge in areas such as crisis intervention, medical services, and case management.
Requirements
  • Direct experience working with individuals experiencing homelessness or complex medical and behavioral health concerns.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and coordination.
  • Valid California driver's license and ability to drive company vehicles.
  • Intermediate level of knowledge of the client population and their complex needs, including homelessness, financial instability, medical and psychiatric illnesses, and substance abuse.
Preferred Qualifications
  • Fluency in other languages.
  • Previous experience or training in street outreach and case management.
  • Knowledge of San Francisco neighborhoods and community resources.
  • Knowledge of the San Francisco Health Provider network.
Work Environment

This position requires frequent travel to various locations, including outdoor settings and occasional work in a general office environment. The ideal candidate will be able to work effectively in a fast-paced environment, with a strong ability to adapt to changing circumstances and priorities.
