Job Summary

We are seeking a highly motivated and compassionate Case Manager Level 1 to join our San Francisco Homeless Outreach Team. As a key member of our team, you will work collaboratively with colleagues to engage and stabilize individuals experiencing homelessness, providing essential support and services to help them achieve permanent housing.

Key Responsibilities
  • Manage a caseload of 15-20 clients with complex medical and/or behavioral issues, developing service plans and linking them to necessary resources.
  • Conduct outreach and engagement activities in various neighborhoods, assessing client needs and providing support and referrals as needed.
  • Collaborate with service providers, community groups, and city agencies to advocate for clients and facilitate access to care.
  • Work collaboratively as part of a dynamic and diverse team, communicating effectively and demonstrating flexibility and humility.
  • Participate in clinical supervision and training to enhance clinical and professional skills.
Requirements
  • High school degree or equivalent plus 2 years of experience working with individuals experiencing homelessness or complex medical and/or behavioral health concerns.
  • Valid California driver's license and a clean driving record.
  • Basic knowledge of client population and their complex needs, including homelessness, financial instability, medical and mental health, and substance abuse.
  • Ability to work well with diverse staff and clientele, including cultural, language, and other diversity considerations.
  • Ability to speak, read, and write clearly in English and basic level competency using the Internet, Outlook email, and Microsoft Word computer applications.
Preferred Qualifications
  • Previous experience or training in street outreach and case management.
  • Knowledge of San Francisco neighborhoods and community resources.
  • Knowledge of the San Francisco Health Provider network.
Work Environment

This position requires frequent travel to various locations, including outdoor settings and occasional work in a general office setting. The ability to stand, walk, climb stairs, sit, drive for extended periods, and lift a minimum of 35 pounds is required.
