Care Manager, Behavioral Health Specialist

SAC Health is seeking a highly skilled Care Manager, Behavioral Health to join our team. As a key member of our Care Management Team, you will be responsible for providing clinical coordination and support to patients with moderate to high-risk behavioral health conditions.
Key Responsibilities- Provide intensive case management for a set caseload of patients with complex behavioral and medical needs.
- Manage both acute and chronic behavioral/mental health diseases, and reevaluate psychosocial plans of care to ensure continued appropriateness.
- Conduct initial screening assessments, ongoing behavioral health and psychosocial assessments, and provide recommendations based on patient needs.
- Monitor for warning signs of decompensation and crisis, and respond accordingly.
- Work collaboratively with interdisciplinary care teams and healthcare practitioners across the continuum.
- Actively participate in outreach and engagement of new patients, and promote programs to patients, families, and staff to build a patient base.
- Meet daily, weekly, and/or monthly productivity expectations set by management.
- Prepare and present systematic case reviews in collaboration with interdisciplinary teams.
- A Master's Degree in Social Work (MSW) or related field of study is required.
- Must have a valid CA Driver's License and Auto Insurance.
- ACSW licensure is highly preferred.
- Current CPR/BLS certification is required.
- 1+ years of experience providing case management and mental health services in a community setting with homeless and/or at-risk populations is highly preferred.
