Job Title: Street Health Outreach Behavioral Health Case Manager

Tiburcio Vasquez Health Center is seeking a highly skilled and compassionate Street Health Outreach Behavioral Health Case Manager to join our team. As a key member of our Street Health Outreach program, you will work directly with participants in conducting behavioral health assessments, providing brief interventions, and supporting clients with navigating mental health and substance use treatment systems from a harm reduction and trauma-informed approach.

Responsibilities:
  • Conduct behavioral health assessments and provide brief interventions to clients
  • Support clients with navigating mental health and substance use treatment systems
  • Maintain a caseload of clients who require more in-depth case management services
  • Provide outreach and engagement, health services, and triage to clients
  • Collaborate with Housing Resource Centers to provide comprehensive services to clients
  • Build relationships with clients and potential clients through consistent visits and outreach
  • Screen and identify individuals with medical and behavioral needs
  • Prioritize individuals with high medical and behavioral health needs and/or Substance Use Disorders for intensive visits
  • Participate in regular meetings with Alameda County Healthcare for the Homeless
  • Provide culturally competent, trauma-informed, and harm reduction-based services
Requirements:
  • High School Diploma (or G.E.D.) required
  • Basic Life Support (BLS)/CPR certification required
  • Valid current driver's license and individual automobile insurance required
  • One year of field experience working with people with serious mental illness, individuals with a dual diagnosis, and/or the homeless population
  • Bilingual in English/Spanish preferred
