Role Summary:


The Therapist - Trans Services is part of a multidisciplinary team that includes medical providers, nurses, medical assistants, social workers, case managers, and community health workers.

The therapist is responsible for providing therapy services to the agency's clients.

This position will carry a caseload of therapy patients and otherwise support the agency in managing the mental health and substance use needs of our clients.

This position will have a special focus on providing behavioral health services to the transgender community within the Trans:

Thrive drop-in center as well as to guests at the Taimon Booton Navigation Center as needed.

Key Responsibilities:
Conduct intake, assessment, and reassessment of clients with mental health and substance use disorders.

Offer individual and group psychotherapy services to clients referred to the agency for behavioral health support in a crisis, brief intervention and long-term capacity.

Conduct Gender Affirming Surgery assessments for Trans and GNC clients, link individuals to necessary supportive services, and coordinate with other providers.

Serve as part of the organization's crisis response team.
Build strong therapeutic rapport creating a trusting and collaborative relationship with clients.
Use skillful verbal and non-verbal communication to effectively listen, reflect, and guide clients.
Always provide a safe and supportive environment for every client.
Update client care plan goals consistently and in a timely manner.
Develop appropriate discharge plans as needed.
Provide a trauma informed, harm reduction approach to care including street-based visits and outreach as needed.
Complete necessary documentation and progress notes and accurately document visits for the complete and timely coding and billing.
Lead and participate in case conferences, monthly staff meetings, morning huddles, consult meetings, and other in-service trainings.
Participate in and comply with agency quality assurance, peer review, clinical consultation.

Work collaboratively with an interdisciplinary team of medical services, medical case management, prevention and testing services, and other social support programs.


Provide onsite consultation to staff within the Trans:
Thrive drop-in center and other programs for urgent behavioral health crisis situations.
Perform other duties and tasks as assigned.

Qualifications:
Master's degree in social work, counseling, marriage & family therapy or equivalent, ASW/AMFT/APCC or equivalent required, LCSW/LMFT/LPCC preferred.
Must possess a high level of emotional intelligence and ability to connect with clients on a deeper level.

Two or more years of direct clinical expertise in assessing and managing the psychosocial needs of low-income populations, multiply diagnosed patients (HIV, mental disorder and substance abuse), transgender/GNC individuals, other sexual and gender minorities, immigrants and refugees.

Experience working in a client-centered environment providing motivational interviewing, harm reduction, and other evidence-based treatment approaches.
Demonstrated experience working collaboratively on interdisciplinary teams.
Proven experience developing and facilitating psychotherapy and support groups.
Extremely adept interpersonal skills, organizational, verbal, and written communication skills.
Demonstrated knowledge of legal mandates and ethical issues related to client privacy and confidentiality.
Ability to travel within San Francisco and, at times, the Bay Area or nationally.
Within six months of employment, successful completion of an Involuntary Hold/5150 certification.
Excellent verbal and written communication in English.
Intermediate skill with MS Suite.
Must obtain TB clearance (within past 12 months) and complete background check upon hire.

Current with vaccinations including latest Covid-19 vaccine with at least one booster, and seasonal flu, and able to provide proof.


Preferred:
Lived experience as a member of priority populations.
Familiarity with data systems such as ARIES, EPIC, electronic health record systems, and Medi-Cal documentation requirements.
Experience as an Enhanced Care Manager.
Bilingual (Spanish strongly preferred).
Active licensure with the California Board of Behavioral Sciences.
Experience working in an FQHC or other type of Community Health Center.
