Job Title: Street Health Outreach Behavioral Health Case Manager

Job Summary:

Tiburcio Vasquez Health Center is seeking a highly motivated and compassionate Street Health Outreach Behavioral Health Case Manager to join our team. As a key member of our Street Health Outreach program, you will work directly with individuals experiencing homelessness and provide essential health and wellness services.

Responsibilities:

  • Conduct behavioral health assessments and provide brief interventions to support clients in navigating mental health and substance use treatment systems.
  • Maintain a caseload of clients who require more in-depth case management services.
  • Provide outreach and engagement services, health services, and triage to support clients in achieving housing stability and improving health and well-being.
  • Build relationships with clients and potential clients through consistent visits and outreach efforts.
  • Screen and identify individuals with medical and behavioral needs, prioritizing those with high medical and behavioral health needs or substance use disorders for intensive visits.
  • Participate in regular meetings with Alameda County Healthcare for the Homeless to ensure seamless coordination of services.
  • Provide culturally competent, trauma-informed, and harm reduction-based services to support clients in achieving their goals.
  • Conduct regular outreach and engagement efforts, building trust with individuals encountered at encampments and other service locations.
  • Respond to immediate survival needs as prioritized by clients, providing basic needs support, problem-solving, health education, and information about health and community services.
  • Provide assessments and administer appropriate screening tools, such as the PHQ2 and PHQ9, and SBIRT services to support clients in achieving their goals.
  • Provide crisis assistance and brief behavioral health interventions, including motivational interviewing and problem-solving therapy, to support clients in achieving their goals.
  • Conduct brief, non-stigmatizing screenings to evaluate the needs of homeless individuals for mental health and support services.
  • Provide behavioral health services utilizing evidence-based treatments, such as brief treatment, problem-solving therapy, and behavioral activation, in a culturally, linguistically, and age-appropriate manner.
  • Provide ongoing consultation for team members and participate in multidisciplinary staff meetings to achieve program development, staff development, and program evaluation goals.
  • Maintain confidentiality regarding privileged administrative and patient information and complete required documentation in a timely manner.
  • Complete Coordinated Entry Assessments for individuals seeking housing.

Requirements:

  • High School Diploma (or G.E.D.) required.
  • Basic Life Support (BLS)/CPR must be obtained prior to start date.
  • Travel to work assignments, a valid current driver's license, and appropriate individual automobile insurance must be maintained.
  • Have a thorough knowledge of the principles and practices of medicine and allied services commensurate with training.
  • Demonstrated ability to build trust and respect with patients, staff, colleagues, external contacts, and other organizational members.
  • Must be able to identify potential escalating situations and safety risks, and be knowledgeable and able to de-escalate.
  • Good problem-solving skills required, including creativity, resourcefulness, timeliness, and technical knowledge related to analyzing and resolving medical problems.
  • Willingness and ability to work some evenings and weekends, as needed, within set time limits.

Qualifications:

  • Bilingual in English/Spanish preferred.
  • Must possess a minimum one (1) year of field experience working with people with serious mental illness, individuals with a dual diagnosis, and/or the homeless population. Experience with street outreach is desirable.
  • Have a working knowledge of psychiatric disorders, as well as knowledge and ability to implement harm reduction, psychosocial rehabilitation, strength-based case management, and motivational interviewing.
