Job Title: Bilingual Community Case Manager

ChenMed is seeking a highly skilled and compassionate Bilingual Community Case Manager to join our team. As a key member of our Community Care team, you will play a vital role in providing exceptional care to our patients and their families.

Job Summary:

The Bilingual Community Case Manager will be responsible for coordinating care for high-risk patients, conducting initial assessments, and developing comprehensive plans of care. You will work closely with our multidisciplinary team to ensure seamless transitions of care and provide education to patients and their caregivers.

Key Responsibilities:
  • Conduct in-home and telephonic visits to assess patient needs and develop care plans
  • Coordinate care with healthcare providers, community resources, and social services
  • Provide education and support to patients and their caregivers
  • Monitor patient progress and adjust care plans as needed
  • Collaborate with the Community Care team to ensure effective care coordination
Requirements:
  • Associate degree in Nursing required; Bachelor's degree in Nursing or related field preferred
  • Valid, active Registered Nurse (RN) license in the state of employment
  • Minimum 2 years of clinical work experience in a hospital setting
  • Minimum 3 years of case management experience in community case management
  • Bilingual in English and Spanish proficiency required
