Our offerings include 24-hour emergency interventions, inpatient psychiatric care for individuals of all ages, and outpatient services such as medication management, case management, and therapeutic counseling.

Job Summary
This role is perfect for individuals who thrive in collaborative environments and are committed to delivering both behavioral health care and social services to community members. The FACT Program comprises a diverse clinical team responsible for providing essential treatment, rehabilitation, and support services to adults experiencing severe and persistent mental health challenges, fostering recovery, empowerment, self-determination, and informed decision-making for each participant.

The FACT team is tasked with delivering clinical and supportive services, which encompass advocacy, crisis intervention, assessments, evaluations, treatment planning, case management, and medication oversight. Participants also receive community referrals, education on symptoms, vocational and educational support, as well as training in social and interpersonal skills, alongside assistance for their families and caregivers.

Most of a FACT team member's time is spent engaging with the community to fulfill these responsibilities, with only a small fraction dedicated to office tasks. While company vehicles are provided, team members are also expected to utilize their personal vehicles for service delivery and are eligible for mileage reimbursement.

The FACT team operates during and after standard business hours, sharing weekend and holiday duties, and rotating responsibilities for carrying an emergency on-call phone.

Key Responsibilities
  • Deliver treatment, rehabilitation, and support services in alignment with the FACT model, adhering to the Program for Assertive Community Treatment (PACT) guidelines.
  • Ensure the accuracy, completeness, and confidentiality of clinical documentation.
  • Conduct mental health assessments to evaluate client needs.
  • Coordinate referrals to community resources and support services.
  • Assist clients with daily living activities, transportation, communication, and relationship-building skills.
  • Provide education on symptoms, medications, and mental health topics.
  • Act as the primary case manager for a designated group of individuals, promoting recovery, empowerment, self-determination, and informed decision-making.
Qualifications
  • A Bachelor's degree in Human Services or a related discipline is required.
  • Prior experience working with adults facing severe and persistent mental health conditions is essential.
  • All FACT team members must meet vehicle driver requirements, including holding a valid driver's license, having adequate automobile insurance, and maintaining a clean driving record.
  • The role involves direct interaction with individuals experiencing serious mental health issues in various settings, including their homes and healthcare facilities.
  • Team members must demonstrate the ability to make sound, independent decisions daily, particularly in emergency situations.
  • Strong communication skills are crucial for effective interaction with clients, families, caregivers, community service providers, and other professionals.
Position Details
This is a full-time position with a schedule from Monday to Friday, including rotating weekends, on-call duties, and holiday responsibilities.
