Mental Health Resource Center is a nonprofit organization dedicated to delivering a diverse array of mental health and behavioral health services to the community. Our offerings include 24-hour emergency assistance, inpatient psychiatric care for various age groups, and outpatient services such as medication management, case management, and counseling.

Job Summary

This position is ideal for individuals who thrive in collaborative environments and are committed to providing essential behavioral health care and social services to community members.

The FACT Program is a multidisciplinary clinical team responsible for delivering necessary treatment, rehabilitation, and support services to adults experiencing severe and persistent mental health challenges. Our focus is on promoting recovery, empowerment, self-determination, and informed decision-making for each participant in the FACT program.

Key responsibilities of the FACT team include delivering clinical and supportive services through advocacy, crisis intervention, assessments, evaluations, treatment planning, case management, and medication oversight. Participants also receive community referrals, education on symptoms, vocational and educational support, and training in social and interpersonal skills, along with assistance for their families and caregivers.

Most of a FACT team member's time is spent in the community fulfilling these responsibilities, with only about 15% allocated to office work. Company vehicles are provided, but team members may also need to utilize their personal vehicles for service delivery and are eligible for mileage reimbursement.

The FACT team operates during and after standard business hours, with responsibilities rotating for holidays and weekends, and team members share an emergency on-call cell phone.

Responsibilities
  • Deliver treatment, rehabilitation, and support services following the FACT model, based on the Program for Assertive Community Treatment (PACT) guidelines.
  • Ensure the accuracy, completeness, and confidentiality of clinical documentation.
  • Conduct mental health status evaluations.
  • Coordinate referrals to community providers and support services.
  • Assist with daily living activities, transportation, communication, and social skills.
  • Provide education regarding symptoms, medications, and mental health topics.
  • Act as the primary case manager for a designated group of individuals, fostering recovery, empowerment, self-determination, and informed decision-making for each FACT participant.
Qualifications
  • A Bachelor's degree in Human Services or a related field is required.
  • Previous experience working with adults facing severe and persistent mental health issues (e.g., schizophrenia, bipolar disorder, borderline personality disorder) is essential.
  • All FACT team members must meet vehicle driver requirements, including a valid driver's license, adequate automobile insurance, and a clean driving record.
  • The role necessitates individual and group interactions with individuals experiencing serious mental health challenges in various settings.
  • Team members must demonstrate the ability to make sound, independent decisions daily, including recognizing emergencies and responding appropriately.
  • Strong communication skills are vital for effective interaction with internal and external stakeholders, including families, caregivers, community service providers, and supervisory staff.
Position Details
Full-Time Position: Monday through Friday (with rotating weekends, on-call duties, and holiday responsibilities).
