OBJECTIVE:

The primary aim of this role is to deliver exceptional nutritional services to both in-center and home patients, enhancing their nutritional health and optimizing clinical results. As a vital member of the interdisciplinary healthcare team, the Dietitian supports the organization's commitment to quality improvement initiatives and actively engages in activities that foster patient-specific goal achievement as outlined by the healthcare provider.

KEY RESPONSIBILITIES:

  • Evaluate patients' understanding of dietary needs related to kidney health and provide tailored education that aligns with their learning preferences.
  • Deliver comprehensive nutritional education to patients, families, and caregivers regarding nutritional status, fluid management, and relevant laboratory results.
  • Conduct thorough nutritional assessments and contribute to interdisciplinary evaluations.
  • Collaborate with the healthcare team to create a personalized care plan that addresses the unique needs of each patient, adhering to regulatory guidelines.
  • Calculate dietary prescriptions in accordance with established practices.
  • Document progress notes for all chronic dialysis patients, ensuring compliance with organizational standards and regulatory requirements.
  • Provide ongoing nutrition assessments and counseling aimed at helping patients achieve and maintain optimal nutritional health.
  • Identify patients at risk of malnutrition and work with the team to implement effective interventions.
  • Monitor patients' adherence to nutritional therapy and adjust plans as necessary to support effective outcomes.
  • Engage with the interdisciplinary team on recommendations for managing mineral metabolism, renal bone disease, anemia, and other related health issues.
  • Participate in quality improvement initiatives and report on nutrition-related outcomes.
  • Coordinate with team members to provide education for patients at risk of end-stage renal disease.
  • Review and interpret nutrition-related laboratory results, advising on necessary actions.
  • Communicate effectively with healthcare providers regarding lab results and treatment recommendations.
  • Attend relevant staff meetings and contribute to discussions on patient care and quality improvement.
  • Consult with corporate dietitians as needed for nutrition-related concerns.
  • Commit to continuous professional development and maintain necessary credentials.
  • Ensure a safe and clean working environment in collaboration with the healthcare team.
  • Perform additional duties as assigned.

WORK ENVIRONMENT:

  • This role is situated within a healthcare facility, characterized by controlled air temperature and moderate noise levels. Exposure to infectious materials may occur.
  • Daily tasks involve desk work, computer usage, and patient interaction.
  • Travel between various facilities and community locations may be required.

EDUCATION AND CREDENTIALS:

  • Registered Dietitian as recognized by the Commission on Dietetic Registration.
  • Board Certification in Renal Nutrition is preferred.
  • Current state licensure as applicable.

EXPERIENCE AND SKILLS:

  • A minimum of one year of professional experience in clinical nutrition as a Registered Dietitian.
  • Previous experience in renal nutrition is advantageous.
  • Demonstrated leadership abilities, including teaching, organizational skills, and effective communication.
  • Ability to work independently with minimal supervision.
  • Basic computer proficiency is required.
