Housing Support Services Coordinator

Position Overview:
LSS of WI & Upper MI is actively seeking a dedicated Service Coordinator to assist our residents in the Northern Wisconsin region.
This role will primarily operate from a central location and will involve travel throughout Northern Wisconsin. The work schedule will be organized to optimize travel efficiency, including provisions for mileage reimbursement and occasional lodging.
Site visits to various locations will be required on a monthly basis, with a structured schedule for other visits determined based on need.
Our initiative is focused on fostering and enhancing the self-sufficiency of individuals living in our properties by facilitating access to a range of supportive services within the community.
Key Responsibilities:
This list of responsibilities is not exhaustive and may be expanded to include additional tasks as deemed necessary by management.
- Provide informal case management, including intake and referral services to tenants requiring assistance when formal case management is unavailable.
- Conduct assessments of health, psychological, and social needs, and develop personalized service plans with periodic evaluations of tenant progress.
- Establish connections with community agencies and service providers to ensure tailored, flexible, and innovative services for tenants.
- Maintain a directory of service providers for use by both staff and tenants.
- Facilitate tenant access to community services such as case management, personal assistance, transportation, and legal advocacy.
- Educate tenants on available services, application processes, and client rights, while providing advocacy as needed.
- Monitor the ongoing delivery of services from community agencies and keep all parties informed of tenant progress.
- Develop volunteer support initiatives in collaboration with local organizations.
- Assist tenants in building informal support networks with fellow residents, family, and friends.
- Provide training to tenants regarding their responsibilities as residents.
- Conduct workshops or informational sessions on topics of interest to tenants.
- Demonstrate the ability to collaborate effectively with tenants, the agency, and the community while also working independently.

Qualifications:
To succeed in this role, candidates must possess the following:
- A Bachelor's degree in social work, sociology, psychology, or a related field, or an equivalent combination of education and experience.
- A minimum of three years of relevant experience, particularly with the older adult population.
- Familiarity with supportive services and resources for older adults and individuals with disabilities is preferred.
Certificates and Skills:
Candidates must have a valid driver's license and reliable transportation. Strong communication skills, both verbal and written, are essential for effective documentation and reporting.
Physical Demands:
This position requires regular travel and may involve exposure to various environmental conditions.
