Job Summary

LSS of WI & Upper MI is seeking a skilled Service Coordinator to provide essential support services to our tenants in the Northern Wisconsin Area.

Key Responsibilities
  • Provide informal case management, including intake and referral services to all tenants needing such assistance.
  • Assess health, psychological, and social needs, and develop individually tailored case plans for services and periodic reassessments.
  • Establish linkages with community agencies and service providers, and set up a directory of providers for use by project staff and tenants.
  • Refer and link tenants to service providers in the general community, including case management, personal assistance, homemaker services, and more.
  • Educate tenants on service availability, application procedures, client rights, and provide advocacy as needed.
  • Develop case plans in coordination with assessment services in the community, and monitor the ongoing provision of services.
  • Set up volunteer support programs with service organizations in the community, and help tenants build informal support networks.
  • Provide training to project tenants on tenancy obligations, and educate other staff on aging in place and service coordination.
  • Present workshops or informative sessions to tenants on topics of interest.

Qualifications
  • Bachelor's degree in social work, sociology, psychology, or related field, or an equivalent combination of education and experience.
  • At least 3 years of related experience, including experience working with the older adult population.
  • Working knowledge of supportive services and other resources for older adults and/or persons with disabilities.
