Key Responsibilities Include:

  • Conduct ongoing evaluations of families to identify psychosocial, financial, environmental, and community resource needs.
  • Collaborate with the care team, healthcare providers, payors, families, and community resources to address care needs and implement the family's support plan.
  • Provide high-quality services as directed by the healthcare provider, including family counseling and caregiver education.
  • Accurately document service delivery daily within our electronic medical record system.
  • Deliver support with empathy, focusing on measurable outcomes.
  • Exhibit enthusiasm in performing at the highest level of licensure.

Qualifications:

  • Possess a Masters or Doctoral Degree from an accredited institution of Social Work.
  • Hold a valid Social Worker License, state driver's license, and auto liability insurance.
  • Have at least one year of experience as a Masters Social Worker in a healthcare, rehabilitation, or home support environment.
