Inpatient Discharge Coordinator

The Inpatient Discharge Coordinator plays a vital role in ensuring a seamless transition of care for patients being discharged from our hospital. This position requires strong communication and organizational skills to coordinate with various healthcare teams and stakeholders.
Key Responsibilities- Participate in daily discharge planning meetings to identify patient needs and develop effective discharge plans.
- Collaborate with inpatient and outpatient case management teams to ensure a smooth transition of care.
- Identify and address barriers to discharge, such as social determinants of health and access to community resources.
- Develop and maintain relationships with community healthcare providers and social services to ensure a comprehensive support system for patients.
- Monitor and report on discharge metrics and quality indicators to inform process improvements.
- Graduate of an accredited School of Nursing (Bachelor's degree preferred).
- Current licensure to practice in the State of Mississippi, free of any encumbrances.
- Registered with the Mississippi State Board of Nurse Examiners.
- At least one year of direct patient care experience.
- Preferred experience in case management.
This role requires moderate physical activity, frequent movement around the office and facility, and occasional travel to other facilities within the SRHS service area. The work involves using repetitive motions, perceiving sound at normal speaking levels, and adjusting vision to bring objects into sharp focus.
Mental Demands- Demonstrate keen mental faculties and assessment and decision-making abilities.
- Exhibit superior communication, speaking, and enunciation skills to receive and give information in person and by telephone.
- Display strong written and verbal communication skills.
- Possess emotional stability conducive to dealing with high stress levels.
- Demonstrate ability to work under pressure and meet deadlines.
