Client Intake Coordinator

Position Title: Intake Specialist
Overview:
CareStar, Inc. is seeking a dedicated Intake Specialist to join our Clinical Department. This role is essential in conducting thorough program evaluations using established tools and methodologies. The Intake Specialist will be responsible for determining eligibility based on program criteria and CareStar standards.
Key Responsibilities:
The selected candidate will be expected to:
- Conduct visits to evaluate eligibility for designated programs and perform safety assessments, adhering to CareStar and departmental protocols.
- Document all interactions and interventions related to applicants in accordance with agency and CareStar policies, ensuring compliance with content and timing standards.
- Utilize the Program Eligibility Assessment Tool (PEAT) and manage all associated documentation as per agency and CareStar guidelines.
- Assist with referrals to community resources as necessary.
- Identify and report high-risk scenarios in line with agency and CareStar procedures.
- Possess an active RN license.
- Have a minimum of 12 months of experience in home and community-based services within the last five years.
- Demonstrate knowledge of Medicare, Medicaid, Home Health Care, and Ohio Home Care waiver programs.
- Adhere to CareStar's standards while fulfilling job duties.
- Understand and comply with CareStar's Policies and Procedures.
- Maintain confidentiality regarding patient information, with strict adherence to privacy regulations.
- Follow the Acceptable Use Policy when utilizing any information systems managed by CareStar, Inc.
