POSITION SUMMARY

The Intake Specialist role at CareStar involves conducting thorough program evaluations using established tools and methodologies. This position is integral to ensuring that individuals receive the appropriate services tailored to their needs.

KEY RESPONSIBILITIES

As an Intake Specialist, you will be expected to:

  • Conduct assessments to determine eligibility for various programs while adhering to CareStar’s guidelines.
  • Document all interactions and interventions related to applicants in accordance with agency policies.
  • Utilize the Program Eligibility Assessment Tool (PEAT) and manage all associated documentation per established protocols.
  • Assist clients in accessing community resources as necessary.
  • Identify and report any high-risk scenarios following agency policies.
  • Maintain an active RN license.
  • Possess a minimum of 12 months of experience in home and community-based services within the last five years.
  • Have a solid understanding of Medicare, Medicaid, Home Health Care, and Ohio Home Care waiver programs.
  • Comply with CareStar’s operational policies and maintain confidentiality regarding patient information.
  • Adhere to the Acceptable Use Policy when utilizing CareStar’s information systems.

WORK ENVIRONMENT

This role primarily serves the Vinton County area and its neighboring regions.
