Position Overview

The role of the Intake Specialist at CareStar is pivotal in enhancing community well-being through effective program assessments. This position is tailored for individuals who are passionate about making a difference in the lives of others and possess the necessary qualifications to evaluate program eligibility and safety.

Key Responsibilities

The selected candidate will be responsible for:

  • Conducting visits to evaluate eligibility for designated programs while adhering to established guidelines.
  • Maintaining comprehensive documentation of all interactions and interventions related to applicants in accordance with agency policies.
  • Utilizing the Program Eligibility Assessment Tool (PEAT) and other relevant documentation as per agency standards.
  • Providing guidance on community resources and referrals as needed.
  • Identifying and escalating high-risk scenarios in compliance with agency protocols.
  • Possessing an active RN license.
  • Having a minimum of 12 months of experience in home and community-based services within the last five years.
  • Demonstrating knowledge of Medicare, Medicaid, Home Health Care, and Ohio Home Care waiver programs is advantageous.
  • Upholding the CareStar Rule in all job-related activities.
  • Understanding and adhering to CareStar's Policies and Procedures.
  • Ensuring confidentiality regarding patient information, with strict adherence to disclosure policies.
  • Following the Acceptable Use Policy when utilizing CareStar's information systems.

Work Environment

This position requires coverage of various counties within the designated area.
