Overview

Halifax Health is a leading healthcare provider seeking a Care Coordination Specialist to join their Care at Home team.

Job Summary

The ideal candidate will possess excellent communication and organizational skills, with the ability to work independently and as part of a team. As a Care Coordination Specialist, you will be responsible for supporting the Office Manager/Administrator/Intake/Billing in ensuring the quality and level of service to our team members exceeds expectations.

Key Responsibilities

  • Patient Care Coordination: Assist in verifying all patient payer sources, including Medicare, Medicaid, Managed Care, and commercial insurance using select payer websites.
  • Order Management: Verify signed/unsigned orders and process eligibility in the Medicare DDE system.
  • Financial Management: Assist in reconciling payer account receivables and balancing daily postings.
  • Human Resources: Ensure adequate staff orientation and knowledge of Human Resources policies and procedures.

Requirements

  • Education: High School Diploma or equivalent required.
  • Experience: Minimum of 2 years in a home health setting, with medical business office experience preferred.
  • Skills: Excellent customer service skills, proficiency in Microsoft Office products and database applications, and accuracy in spelling, grammar, and communication.

Compensation

We offer a competitive salary range of $65,000 - $80,000 per year, depending on experience, plus benefits and opportunities for professional growth and development.
