As a Clinical Care Coordinator with Carilion Clinic, you will play a vital role in ensuring the highest level of patient care and satisfaction. This position is responsible for coordinating and managing the care of patients in their homes, hospitals, and other healthcare settings. You will work collaboratively with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient.

In this role, you will be responsible for conducting comprehensive assessments of patients' physical, emotional, and social needs, as well as identifying opportunities for education and support. You will also facilitate communication between patients, families, caregivers, and healthcare providers to ensure seamless transitions of care. With a strong emphasis on teamwork and collaboration, you will work closely with our nursing staff, physicians, and other healthcare professionals to deliver high-quality patient-centered care.

We offer a competitive salary of $65,000 - $85,000 per year, depending on experience. As a valued member of our team, you will also receive a comprehensive benefits package, including medical, dental, and vision insurance, as well as paid time off and holidays.
