Job Summary

The Clinical Care Coordinator will be responsible for coordinating patient care services, ensuring seamless transitions between healthcare providers, and promoting overall patient well-being.

About the Role
  • Develop and implement patient care plans, taking into account individual needs and preferences
  • Collaborate with healthcare teams to ensure effective communication and coordination of care
  • Monitor patient progress, identifying areas for improvement and implementing adjustments as needed
  • Provide education and support to patients and their families, empowering them to take an active role in their care
Requirements

To be successful in this role, you will need:

  • A current Registered Nurse (RN) license for the state in which you practice
  • One year prior RN experience preferred
  • Excellent communication and interpersonal skills
  • Able to work independently and as part of a team
