Job Description

The RN Case Manager-Home Health position at Carilion Health System involves interacting with patients, families, and healthcare teams to develop, coordinate, and manage patient care plans. The successful candidate will make home visits, provide support to other staff, and ensure complete hand-off of information.

Required Skills and Qualifications

To be considered for this role, applicants must have a graduate degree in nursing, one year of experience, and a valid Virginia driver's license. Current licensure as a Registered Nurse in the Commonwealth of Virginia or eligibility to sit for the Virginia licensure exam is also required.

This full-time position is located at Carilion Clinic Home Care-Lexington, with a shift of Monday-Friday 8am-4:30pm, including holiday and on-call responsibilities.
