Community Resource Liaison

We are seeking a skilled Resource Navigator to join our team at Waypoint. In this role, you will play a critical part in supporting the well-being of our clients by connecting them with essential resources within their community.
About the Role
- As a Resource Navigator, you will serve as a bridge between our organization and the community, providing hands-on assistance and advocacy to help clients overcome barriers to accessing resources.
- You will work closely with clients, families, and caregivers to identify and address their unique needs, creating personalized plans to promote independence and well-being.
- In this role, you will also have the opportunity to work collaboratively with our team to develop and implement innovative programs and services that meet the evolving needs of our community.
Requirements
- Bachelor's degree in social work or a related field.
- At least 2 years of experience in a related field, with a focus on working with elderly or adults with disabilities.
- Successfully pass a criminal background check and possess a valid driver's license with proof of insurance.
