Welcome to the Elliot Health Systems team, where you can make a meaningful difference in the lives of patients and families. We are seeking an experienced Nurse Case Manager to join our Care Coordination Department at the Elliot Hospital.

As a key member of our health care team, you will collaborate with other professionals to identify appropriate utilization of resources and create effective transition plans for our patients. With your strong clinical skills and experience, you will assess patient needs, provide resources and community referrals, and coordinate care with families, providers, and clinical teams.

We offer a competitive salary of approximately $ 85,000 per year, based on industry standards and location considerations. The ideal candidate will have a Bachelor's degree in a healthcare-related field or equivalent experience, with 3-5 years of case management-related health care experience. A CCM or ACM certification is preferred, as well as excellent clinical documentation and EMR system usage skills.
