About the Role
The Patient Care Navigator RN will collaborate with other members of the healthcare team to identify appropriate utilization of resources and develop a safe and effective transition plan. This role requires strong clinical experience and skills, with a focus on creating a personalized care plan for each patient.

Key Responsibilities
* Assess and identify patients' needs during the discharge planning process
* Provide resources and community referrals for continuity of care
* Coordinate care with families, providers, and the clinical team
* Follows Medicare conditions when needed
* Conduct utilization reviews

Requirements
* Bachelor's degree in a healthcare-related field or equivalent experience
* Current BSN or commitment to achieving the degree in a specified amount of time

* 3 - 5 years of case management-related healthcare experience
* Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification preferred
* Excellent clinical documentation and EMR system skills required
