Job Summary

CAN Community Health is seeking a skilled Licensed Practical Nurse to join our team in Sarasota, FL. As a key member of our multidisciplinary team, you will provide high-quality patient care under the supervision of a Medical Provider and Registered Nurse.

Key Responsibilities
  • Provide basic nursing care to patients, including vital sign assessment, medication administration, and wound care.
  • Perform patient triage, including history taking and review of patient charts to ensure accurate documentation.
  • Process medication refills and perform venipuncture, obtaining specimens as needed.
  • Collaborate with the care team to ensure seamless patient care and coordinate services with other departments and healthcare facilities.
  • Maintain accurate and timely documentation of patient care, adhering to HIPAA standards and grant requirements.
Requirements
  • LPN/LVN graduate of an accredited school of nursing, currently licensed to practice in the state of Florida.
  • Basic Cardiac Life Support (CBL) Certification.
  • Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Proficient in using electronic medical records and computer software programs.
Work Environment

This position requires the use of standard nursing equipment and supplies, including medical equipment and devices. The successful candidate will be able to work in a fast-paced environment, prioritizing tasks and managing time effectively to meet the needs of patients and the care team.
