Licensed Practical Nurse Level 1

CAN Community Health is seeking a highly skilled Licensed Practical Nurse Level 1 to join our team of healthcare professionals. As a key member of our clinical staff, you will provide high-quality patient care and support to individuals impacted by HIV and other infectious diseases.
Key Responsibilities- Patient Care: Provide basic nursing care to patients under the direct supervision of a Medical Provider (MD, PA, APRN) and Registered Nurse (RN).
- Team Collaboration: Work as part of a multidisciplinary team to enhance patient quality of life through the provision of nursing care.
- Medication Management: Process medication refills per medical provider's order and in compliance with CAN Medication refill and EMR Policies.
- Lab Procedures: Perform venipuncture, obtain specimens, and process these according to laboratory specific requirements.
- Documentation: Document all patient care in an accurate and timely manner, and ensure compliance with grant requirements (340B, HRSA, Ryan White).
- Education: LPN/LVN graduate of an accredited school of nursing currently licensed to practice in the state where clinic is located.
- Certifications: Basic Cardiac Life Support (CPR) Certification.
- Communication: Maintain strong interpersonal and communication skills while working with patients, visitors, and clinical team.
- Computer Skills: Proficient ability to use a computer and electronic medical record. Possess the ability to learn and utilize computer software programs.
- Confidentiality: Maintain patient, team member, and employer confidentiality. Comply with all HIPAA regulations.
- Competent Nursing Skills: Ability to perform basic nursing skills safely and effectively.
- Customer Service Oriented: Friendly, cheerful, and helpful to patients and others. Ability to meet patients' and others' needs while following CAN policies and procedures.
- Detail Oriented: Ability to pay attention to the minute details of a project or task.
- Adaptability/Flexibility: Ability to adapt easily to changing conditions and work responsibilities.
- Interpersonal Skills: Display a positive attitude and is a positive agent for change.
- Teamwork: Work as part of a team and collaborate with co-workers.
- Problem-solving/Decision Making: Ability to identify problems and make decisions under stressful situations.
