Job Summary

CAN Community Health is seeking a highly skilled Licensed Practical Nurse Level 1 to join our team of healthcare professionals. As a Licensed Practical Nurse Level 1, you will play a vital role in providing high-quality patient care and support to our clients.

Key Responsibilities
  • Patient Care: Provide basic nursing care to patients under the supervision of a Medical Provider (MD, PA, APRN) and Registered Nurse (RN).
  • Team Collaboration: Work as part of a multidisciplinary team to enhance patient quality of life through the provision of basic nursing care.
  • Medication Management: Process medication refills per medical provider's order and in compliance with CAN Medication refill and EMR Policies.
  • Lab Procedures: Perform venipuncture, obtain specimens, and process these according to laboratory specific requirements.
  • Documentation: Document all patient care in an accurate and timely manner, and ensure compliance with grant requirements (340B, HRSA, Ryan White).
Requirements
  • Education: LPN/LVN graduate of an accredited school of nursing currently licensed to practice in the state where clinic is located.
  • Certifications: Basic Cardiac Life Support (CPR) Certification.
