Cancer Screening Patient Navigator Job Description

Westside Family Healthcare is seeking a dedicated and compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to fill the role of Cancer Screening Patient Navigator. This position is a contracted position funded by the Delaware Division of Public Health.

The Cancer Screening Patient Navigator will play a vital role in guiding individuals through the cancer-screening process. This includes educating eligible individuals about cancer screening and associated processes, scheduling screening appointments, linking patients with cancer screening programs such as Screening for Life, and eliminating barriers to screening.

The ideal candidate will have experience using computers, Microsoft Word, Excel, and Outlook, as well as electronic medical records software. A background in health sciences or a related discipline is also preferred.

Responsibilities:

  1. Identify patients in need of cancer screening and guide them through the process.
  2. Provide education and support regarding the need for cancer screening.
  3. Serve as an advocate for patients and providers by determining and meeting their needs.
  4. Implement activities that encourage individuals to receive cancer screenings.
  5. Maintain patient confidentiality and discuss personal health information only with authorized personnel.

Requirements:

  1. Registered Nurse or Licensed Practical Nurse with current Delaware licensure.
  2. Associate's Degree in nursing, health sciences, or a related discipline.
  3. Experience using computers and electronic medical records software.
  4. Experience as a trainer or educator.
