Job Title: Patient Navigator

SUNY Downstate Health Sciences University is seeking a full-time Patient Navigator to join our team in Ambulatory Care Services. The successful candidate will provide pre-visit review of patients' charts, communicate with providers, coordinate preventive care services, and track patients' progress through health education curriculum.

Key Responsibilities:

  • Provide pre-visit review of patients' charts and communicate with providers
  • Coordinate all preventive care services and review charts for quality improvement
  • Track patients' progress through health education curriculum and reinforce and coach as needed
  • Provide critical feedback to healthcare team members through EHR notes and messaging
  • Participate in multi-disciplinary care conference meetings and phone communications

Requirements:

  • High School Diploma
  • 1-2 years of experience
  • Strong interpersonal, verbal, and written communication skills

Preferred Qualifications:

  • Bachelor's Degree (or working towards a Bachelor's Degree)
  • Bilingual in Haitian, Creole, or Spanish language is a plus

Work Schedule: Monday to Friday, 9:00am to 5:00pm (Full-Time)

Salary Grade/Rank: SL-1

Salary Range: Commensurate with experience and qualifications
