Patient Navigator

Join SUNY Downstate Health Sciences University as a Patient Navigator and play a vital role in ensuring seamless patient care. As a key member of our Ambulatory Care team, you will be responsible for providing pre-visit reviews, coordinating preventive care services, and tracking patient progress through our health education curriculum.
Key Responsibilities:- Provide pre-visit reviews of patient charts and communicate with providers to ensure seamless care coordination.
- Coordinate preventive care services, including screenings and referrals, to promote patient health and well-being.
- Track patient progress through our health education curriculum and provide coaching and reinforcement as needed.
- Participate in multi-disciplinary care conferences and phone communications to ensure effective patient care.
- Identify and resolve barriers to patient care, including social determinants and transportation issues.
- High School Diploma or equivalent.
- 1-2 years of experience in a healthcare setting.
- Strong interpersonal, verbal, and written communication skills.
- Bachelor's Degree or working towards a Bachelor's Degree.
- Bilingual in Haitian, Creole, or Spanish language is a plus.
