Job Overview

Aveanna Healthcare is seeking a compassionate and skilled Pediatric Home Health Care Nurse to provide high-quality nursing care to patients in their homes. As a Licensed Practical Nurse (LPN) with Aveanna, you will have the opportunity to make a meaningful difference in the lives of children and families.

Responsibilities
  • Provide one-on-one patient care in a home environment, executing physicians' prescribed plans of care and documenting care provided in a timely manner.
  • Administer prescribed medication, treatments, and therapies, and coordinate care with other healthcare professionals as needed.
  • Assess patient needs, develop and implement individualized care plans, and educate patients and families on disease management and prevention strategies.
  • Collaborate with the healthcare team to ensure seamless care transitions and optimal patient outcomes.
Requirements
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
