Pediatric Home Health Care Nurse Job Description

Aveanna Healthcare is seeking a compassionate and skilled Pediatric Home Health Care Nurse to provide high-quality nursing care to patients in their homes. As a Pediatric Home Health Care Nurse, you will work closely with our team to deliver personalized care and support to patients and their families.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, under the supervision of a Registered Nurse.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical and emotional needs, and document all care provided.
  • Administer medications, treatments, and therapies as prescribed by physicians.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Provide education and support to patients and families on disease management, self-care techniques, and prevention strategies.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
