POSITION OVERVIEW

The RN Case Manager Lead will report directly to the RN Manager of Case Management and will oversee the team of RN Case Managers across various departments. The ideal candidate will demonstrate strong leadership capabilities, effective communication skills, and adept problem-solving abilities. This role is crucial in ensuring the provision of high-quality, patient-centered care that prioritizes the patient experience. The RN Case Manager Lead will also contribute significantly to program development, staff training, and community engagement.

KEY RESPONSIBILITIES

Responsibilities include but are not limited to:

  • Assisting the Director of Case Management and Social Work Manager in enforcing departmental policies and procedures.
  • Supervising, orienting, and mentoring RN Case Managers.
  • Ensuring compliance with documentation standards for assessments and discharge notes.
  • Facilitating comprehensive discharge planning tailored to patient and family needs.
  • Identifying service gaps and developing strategies to enhance patient care and resource use.
  • Engaging in quality improvement initiatives related to care standards and patient outcomes.
  • Providing resources to address patients' social and emotional needs.
  • Ensuring safe transitions for patients between care settings.
  • Encouraging patient and family involvement in discharge planning.
  • Collaborating with multidisciplinary teams and community partners for holistic patient care.
  • Maintaining up-to-date knowledge of case management principles and regulatory requirements.
  • Evaluating program effectiveness and suggesting improvements based on feedback.
  • Offering direct support for complex cases and participating in case reviews.
  • Fostering a collaborative and efficient team environment.
  • Working with social work partners and the multidisciplinary team to address patient needs.

QUALIFICATIONS

To be considered for this role, candidates should possess:

  • A minimum of an Associate Degree in Nursing; a BSN is preferred.
  • At least 2 years of supervisory or managerial experience in hospital Case Management.
  • A valid Registered Nurse license in the state of Florida.
  • Basic Life Support (BLS) certification from a recognized provider.
  • Preferred Case Management Certification.
  • Intermediate proficiency in Microsoft Office applications.
