Key Responsibilities- Provide medical case management services to individuals through in-person and telephonic communications with patients, physicians, and other healthcare providers.
- Utilize your medical and nursing knowledge to discuss treatment plans with physicians and explore alternative options.
- Evaluate treatment plans for appropriateness, medical necessity, and cost-effectiveness.
- Assess, plan, implement, and evaluate patient progress.
- Attend medical appointments, hospital visits, and other relevant meetings to ensure effective care.
- Implement care strategies, including negotiation of durable medical equipment and nursing services.
- Travel locally to meet with patients and healthcare providers, with occasional overnight travel.
- Graduate of an accredited school of nursing.
- Current RN licensure in the state of operation.
- Certification as a CCM, CIRS, or other case management certifications preferred.
- Valid driver's license, reliable transportation, and ability to travel to assigned locations.
