POSITION SUMMARY

The RN Case Manager Lead will report directly to the RN Manager of Case Management and will oversee the team of RN Case Managers across various departments. The ideal candidate will demonstrate strong leadership abilities, effective communication skills, and a knack for problem-solving. This role is pivotal in ensuring the delivery of high-quality, patient-centered care while also focusing on enhancing patient experiences. The RN Case Manager Lead will contribute significantly to program development, staff training, and community engagement.

KEY RESPONSIBILITIES

Additional responsibilities may be assigned as needed.

· Support the Director of Case Management and Social Work Manager in the execution and adherence to organizational policies and procedures.

· Supervise, orient, and mentor RN Case Managers.

· Ensure compliance with documentation standards for assessments and care notes.

· Facilitate individualized discharge planning tailored to patient and family needs.

· Identify service gaps and develop strategies to enhance patient care and resource utilization.

· Engage in quality care initiatives aimed at reducing length of stay and readmissions.

· Provide resources to patients and families to address their social and emotional healthcare needs.

· Ensure safe transitions for patients between care environments.

· Advocate for patient and family involvement in discharge planning processes.

· Collaborate with multidisciplinary teams and community partners to ensure comprehensive patient care.

· Maintain an in-depth understanding of case management principles, healthcare practices, and regulatory standards.

· Stay informed on evidence-based best practices in case management and integrate new approaches into team workflows.

· Evaluate program effectiveness and recommend enhancements based on data and feedback.

· Provide direct support for complex cases, participating in weekly case reviews.

· Foster a collaborative and supportive team culture.

· Assist with complex cases, including guardianship and care for underserved populations.

· Work with the utilization management team to ensure timely regulatory compliance and appropriate care levels.

· Collaborate with social work partners and the multidisciplinary team to optimize patient care.

EDUCATION AND EXPERIENCE REQUIREMENTS

· Associate Degree in Nursing required; BSN preferred.

· Minimum of 2 years of supervisory or managerial experience in hospital case management.

· Current Registered Nurse license in the state of Florida.

· Basic Life Support (BLS) certification required.

· Case Management Certification preferred.

· Proficient in Microsoft Office Suite, including Word, Excel, and Outlook.
