POSITION SUMMARY

The RN Case Manager Lead will report directly to the RN Manager of Case Management and will oversee the team of RN Case Managers within the inpatient and emergency departments across all NCH campuses. The ideal candidate will demonstrate strong leadership capabilities, effective communication skills, and adept problem-solving abilities. This role will serve as a key resource for RN Case Managers, ensuring the provision of high-quality, patient-centered care that prioritizes patient experience. Additionally, the RN Case Manager Lead will be instrumental in program development, staff training, and community engagement.

KEY RESPONSIBILITIES

Other responsibilities may include:

  • Assisting the Director of Case Management and Social Work Manager in the implementation and adherence to NCH and departmental policies and procedures.
  • Supervising, orienting, and mentoring RN Case Managers.
  • Ensuring compliance with departmental documentation standards, including comprehensive assessments and discharge notes.
  • Facilitating individualized discharge planning that addresses the needs of patients and their families.
  • Identifying service gaps and developing strategies to enhance patient care and resource utilization.
  • Promoting quality of care initiatives and managing metrics related to length of stay and readmissions.
  • Providing resources to patients and families to address their medical and emotional needs.
  • Ensuring safe transitions for patients between care environments.
  • Encouraging patient and family involvement in discharge planning.
  • Collaborating with multidisciplinary teams and community partners to ensure comprehensive patient care.
  • Maintaining knowledge of case management principles, healthcare practices, and regulatory requirements.
  • Staying informed on evidence-based best practices in case management and integrating new approaches into the team's workflow.
  • Evaluating program effectiveness and suggesting improvements based on data and feedback.
  • Providing direct support for complex cases and participating in weekly case reviews.
  • Fostering a collaborative and efficient team culture.
  • Working closely with social work partners and the multidisciplinary team.
  • Assisting with staffing as necessary.

EDUCATION AND EXPERIENCE REQUIREMENTS

• Minimum of an Associate Degree in Nursing required; BSN preferred.

• At least 2 years of supervisory or managerial experience in hospital Case Management is required.

• Must be a licensed Registered Nurse in the state of Florida.

• Basic Life Support (BLS) certification is required from a recognized provider.

• Case Management Certification is preferred.

• Proficient in Microsoft Word, Excel, Outlook, and Windows.
