We are seeking a highly skilled and compassionate Case Manager to join our team at Community Health Network. As a Case Manager, you will play a critical role in ensuring the seamless transition of patients from one level of care to another, while promoting quality patient outcomes and efficient utilization of healthcare services.
Key Responsibilities- Conduct comprehensive assessments of patient/family goals and biophysical, psychosocial, environmental, financial, and discharge planning needs.
- Develop and implement goal-directed plans of care, prioritized and based on intermediate goals and specific outcome criteria.
- Coordinate and facilitate care in a knowledgeable, skillful, and consistent manner.
- Collaborate with nursing staff and other ancillary patient care departments to ensure smooth transitions and address barriers or delays.
- Participate in quality improvement initiatives and contribute to the development and evaluation of care management team functions.
- LCSW or LSW licensure in the State of Indiana.
- Master's Degree in Social Work.
- 2+ years of inpatient, outpatient, or home-base social work experience.
- Experience with quality improvement methodology preferred.
