Job Summary

We are seeking a highly skilled and experienced Home Health Clinical Manager to join our team at Providence. As a Home Health Clinical Manager, you will be responsible for coordinating the multidisciplinary home health services for our clinical team.

Key Responsibilities

  • Provide communication, information, and tools necessary to maintain best practices in all team roles.
  • Act as a liaison between Access Data Entry, Scheduling, Quality, and clinical Staff.
  • Have access to confidential organizational financial data pertinent to OASIS data integrity to compile data and/or reports utilizing both workflow and financial planning information.
  • Collaborate with the clinical team to ensure seamless delivery of home health services.
  • Develop and implement strategies to improve patient outcomes and satisfaction.

Requirements

  • Bachelor's Degree in a healthcare-related field.
  • Washington Registered Nurse License.
  • National Provider BLS - American Heart Association.
  • National Provider Identifier (NPI) number.
  • Three years of experience in the nursing or rehab field.

Preferred Qualifications

  • Case Management experience.
  • Supervisory experience in Home Health.
