Responsibilities- Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
- Collaborate with the interdisciplinary team to coordinate care and ensure timely interventions.
- Administer medications and treatments as prescribed by the physician, and provide education to patients and families on self-care and disease management.
- Communicate effectively with patients, families, and healthcare providers to ensure a coordinated and patient-centered approach to care.
- Ensure compliance with all state and federal regulations, and maintain accurate and timely documentation of patient care.
- Graduate of an accredited nursing program with current licensure to practice in the state of operation.
- At least one year of nursing experience, with a preference for experience in home health or a similar setting.
- Strong commitment to clinical excellence and patient-centered care.
