This exciting opportunity is designed for an experienced nurse who can provide health education on an individual and group level, utilizing analytic data to improve employee engagement, affordability, and decision-making. You'll empower members to make better healthcare decisions and connect them with the appropriate resources.
Key Responsibilities- Educate members on health education and condition management, collaborating with OptumHealth to provide formal and informal education, as well as plan specifics and member resources.
- Refer employees to available clinical programs, including premium designated physicians.
- Create, manage, and implement employer-specific programs based on employer needs.
- Monitor and analyze customer-specific health analytic data, developing targeted data-driven strategic plans and programs, and providing support to clinical programs.
- Clinical interface with facilities and providers for resolution of issues concerning members, benefit interpretation, program definition, and clarification.
- Knowledge of community resources for referral and community outreach.
- Registered Nurse with an active and unrestricted license for the state of residence.
- 3+ years of clinical experience in a hospital, acute care, home health/hospice, direct care, or care management.
- Solid presentation skills with proven experience delivering group presentations and 1:1 coaching.
- Proficient in Microsoft Word, PowerPoint, and Excel, including the ability to navigate a Windows environment.
- Bachelor's degree in Nursing.
- Case management experience and/or Case Management Certification (CCM).
- 3+ years of experience in managed care with significant program management experience.
- Comfortable with individual and group presentations up to 50-100.
- Previous wellness/disease management, program promotion, education, and development experience.
- Diabetes education experience.
- Community outreach experience.
- Possess familiarity in data analysis (utilization/program outcomes) and reporting.
