We are seeking a skilled and compassionate LPN Home Care Specialist to join our team at Specialty Home Care.
About the Role
- Provide high-quality, patient-centered care to medically fragile patients in their homes.
- Work collaboratively with our interdisciplinary team to develop and implement individualized care plans.
- Conduct thorough assessments, develop and implement care plans, and provide ongoing care and support to patients.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements
- 1-2 years of hospital experience, with a focus on acute care.
- Hands-on experience with tracheotomy, in-home ventilator care, nebulizer management, G-tube management, and Foley care management.
- Reliable transportation and good documentation skills.
- Current, unencumbered license to practice as a LPN or RN in the District of Columbia.
- Current CPR certification and a copy of updated PPD/Chest X-Ray.
